SCFT's Palliative and End of Life Care Service supports patients in their last year of life to live as comfortably as possible at home. The service have recently launched an innovative Digital Comfort Tracker app to allow patients and carers to log symptoms from home, which are then
reviewed by health care teams on a central dashboard. The app also offers guidance on managing symptoms, links to trusted resources, and a sense of connection for patients who may otherwise feel isolated.
Volunteers are required to help out with the roll out of this app, by contacting patients to:
- Explain what the app is and how it can help at home.
- Gain the patient's consent/their carer's consent to use the app.
- Obtain required patient information for healthcare professionals.
We have vacancies at each of the following locations: Burgess Hill, Crawley, East Grinstead, Haywards Heath, Lewes.
Skills
- Excellent telephone, communication and listening skills.
- The ability to be patient when speaking to individuals who may be upset, feeling unwell or in pain.
- Able to identify when information needs to be repeated and do this in a clear and supportive manner.
- Able to fill in a patient information sheet with clear and neat handwriting
NB: This role is not suitable for people aged under 18 and those currently caring for a loved one in their last year of life or those who have experienced a bereavement in the last 2 years.

The role is for one four hour shift per week, time to be negotiated but to be between 9am and 4pm Tuesday, Wednesday or Thursday.
